Doors, turnstiles, visitor flow and an audit trail — with permissions that are actually revoked when somebody leaves.
Corporate access control is less about the hardware than about the process behind it. The technology reliably records who went where; what fails is the administration, and specifically revocation. Permissions accumulate as people change roles and are rarely removed, so after a few years the system's answer to who can enter a server room is longer than anybody expects.
That makes the joiner-mover-leaver process part of the design rather than an operational afterthought. Where access control is linked to the HR or identity system, a leaver is revoked because the payroll record changed; where it is not, revocation depends on somebody remembering, and the audit trail slowly stops meaning anything.
Reception and visitor flow is the other half. A visitor process built on a paper book produces no usable record and no evacuation list, and in a multi-tenant building it is often the landlord's requirement rather than an option.

If the answer is manually, permissions are almost certainly stale. Linking to HR or the directory is the fix.
